Sign up for a time to get a Flu Shot at one of UIndy’s Flu Shot Clinics.

Please read over this important information before your appointment:

When arriving at the clinic:

1. Bring a pen.
2. Wear a shirt that will allow for the injection to be given in the upper arm.
3. Bring a signed consent form (will be sent prior to your appointment).
4. Wear a mask.
5. Review the Vaccine Information Sheet (will be sent prior to your appointment).

Clinics will take place in the following locations:

• Tuesday, October 27th – The ARC
• Thursday, October 29th – Tent Outside Schwitzer
• Tuesday, November 3rd – The ARC
• Thursday, November 5th – Test Outside Schwitzer
• Tuesday, November 10th – The ARC
• Thursday, November 12th – Tent Outside Schwitzer
